    PROJECT SUMMARY/ABSTRACT Trans women experience a profound and persistent sexual health disparity, with an estimated global HIV prevalence of ~19% (nearly 50 times higher when compared to adults of reproductive age) and substantial burdens of non-HIV sexually transmitted infections (STIs), including Chlamydia trachomatis (2.7–24.7%), Neisseria gonorrhoeae (2.1–19.1%), and syphilis (1.4–50.4%). Despite this burden, non-HIV STIs remain critically understudied in this population, with limited laboratory-confirmed data, scarce evidence on post genital surgery tissue STI risk, and no standardized multisite screening guidelines. Multilevel factors, including healthcare access limitations and variability in patient–provider interactions, contribute to low and inconsistent STI testing, undermining effective prevention and treatment. This study addresses these critical gaps by evaluating scalable STI testing strategies and generating robust epidemiologic data. Aim 1 will assess the acceptability, feasibility, and validity of self-collected versus clinician-collected genital and non-genital specimens for STI detection among trans women using mixed methods. Aim 2 will estimate multisite (genital, rectal, pharyngeal) STI prevalence. Aim 3 will characterize the influence of social and spatial network factors (e.g., network structure, social support, neighborhood context) on STI risk, including differences by surgical status. By integrating biologic, behavioral, and socio-spatial approaches, this project will generate critical evidence to inform STI screening guidelines and intervention strategies, ultimately improving access to testing, enhancing early detection, and reducing STI disparities.
